Home health care services provided by Corus Health are ordered by your physician and provided by our team of skilled nursing professionals, therapists and/or trained attendants. Because we have over 100 years of experience across many levels of healthcare, we can provide the best quality care for each individual patient, no matter what those specific needs might be.

We offer programs such as Palliative/Pain Management, Falls Prevention, Breathing Re-education, Incontinence, Electrical Stimulation for Swallowing, Neurological Rehab, Lymphedema Management and more.


Our professional services include:

  • Skilled Nursing | Our staff of registered nurses help with a patient’s journey toward recovery and better quality of life from the day of discharge to treatment at home and in helping a patient gain independence in managing their disease with the skilled medical care a patient needs. These services may include pain management and medication administration through I.V. therapy, caring for wounds or aid with medical equipment, monitoring a patient’s general health and providing other types of health support.
  • Physical Therapy | Physical therapists are a valuable part of our home health team, as they help a patient regain the use of their muscles and joints and also train our non-medical attendants and clinicians to be able to continue those therapies with the patient in their home on a daily basis.
  • Occupational Therapy | An occupational therapist can help a patient with physical, developmental, social or emotional disabilities as well as teach them to perform daily functions like eating, bathing, dressing and more. An invaluable resource to the Corus Health team, they also aid in the training of our non-medical attendants and clinicians so that a patient can continue those therapies in their home on a daily basis.
  • Speech Therapy | A speech therapist can help a patient regain the ability to communicate, aiding in that patient’s journey toward independence in managing their disease at home.
  • Home Health Aides | Our personalized services provide what you need to remain living at home. There to meet your specific needs, personal care attendants are trained by nurses and physical and occupational therapists to assist patients with monitoring vital signs, participating in activities of daily living and ensuring safety in the home. Services may include mobility and orientation, personal hygiene, social interaction, simple errands and more.

The Professional Services provided by Corus Health are part of the Medicare program called Transitional Care Management, a program created in 2013 to help patients manage their healthcare needs following a discharge from an Inpatient Acute Care Hospital, Skilled Nursing Facility, Long Term Care Hospital, Inpatient Rehabilitation Facility or Hospital, outpatient observation or partial hospitalization.
